If the events that a student participates in changes, please revise this form on-line by registering for on-line access using the link on the email you will receive, if you have not done this before. There are a lot of changes that happen at the deadline, it is fine if this is completed at the last minute to minimize the changes you have to make.
You will receive a copy of this form via email when you click on submit.
Please enter the events below one line per row to make it easier for the form to be copied and distributed to the judges. For team events, please include the team number that this student will be in.
There is no need to include pre-convention challenge events, since churches will handle those events locally, and just send us the medal counts awarded.
